The California Healthy Kids Survey (CHKS) is an anonymous, confidential survey of school climate and safety, student wellness, and youth resiliency. It is administered to students at grades five, seven, nine, and eleven. It enables schools and communities to collect and analyze data regarding local youth health risks and behaviors, school connectedness, school climate, protective factors, and school violence. The CHKS is part of a comprehensive data-driven decision-making process on improving school climate and student learning environment for overall school improvements.
 
 
Your student's school will be sending additional information regarding survey dates and opt-in or opt-out deadlines.
  • Most participating sixth-graders will take the survey the week of 2/27/23
  • Most survey administration for grades 7-12 will be the week of 3/6/23 unless a student provides an opt-out form.

Survey Content. The survey gathers information on developmental supports provided to youth; school connectedness and barriers to learning; school safety; health-related concerns such as physical activity and nutritional habits; alcohol, tobacco, and other drug use; risk of depression and suicide; and perceived sexual orientation.

The results from this survey are compiled into district- and county-level CHKS Reports. To view a copy of your district’s report, go to https://calschls.org/reports-data/search-lea-reports and type in the district name.
 
It is Voluntary. Students who, with your permission, agree to participate do not have to answer any questions they do not want to answer, and may stop taking the survey at any time.
 
It is Anonymous. No names are recorded or attached to the survey forms or data. The results will be made available for analysis only under strict confidentiality controls.
 
Administration. The survey will be administered on-line, sometime during the months of November and December  2020, and will take less than one class period to complete (under 50 minutes).
 
Potential Risks. There are no known risks of physical harm to your child. Risks of psychological or social harm are very small. None have been reported in 21 years of survey administration. In rare instances, some discomfort might be experienced from the questions. The school’s counseling services will be available to answer any personal questions that may materialize.

CA Youth Tobacco Survey

CA Youth Tobacco Survey - Administered to Students in Grades 8th, 10th and 12th

Survey Content from the CA Department of Public Health's CA Tobacco Control Program and the CDE: "What: Student will be asked about their tobacco-related knowledge, attitudes and behavior; marijuana use and mental health; and participation in school and community tobacco prevention programs. The survey is online, only takes about 30 minutes, and will be completed at school. The survey is anonymous; your child's name will not be attached to their answers. Your child's answers will be confidential; the school will not see your child's answers, and no one will have any way of knowing who completed each survey." The survey may include protected class identifiers such as sexual orientation and gender identity.
  • The CA Youth Tobacco Survey will be administered to all 8th, 10th and 12th grade students unless a parent/guardian submits a signed parental form to opt out of the survey participation.
  • Parents/Guardians can visit the CA Youth Tobacco Survey website at https://cyts.rti.org/
  • Parents/Guardians who want to view the survey can contact CYTS@rti.org.
  • Parents/Guardians who do not want their child to participate in the survey must return the opt-out form to their student's school.
